
SERVICE OFFER
In a few cases, providers have patients that may start off doing well on treatment and begin to drop off right before compliance is met, placing meeting compliance at risk.
There are times when a patient is aware of how treatment works and is in need of another device; therefore, the provider is only interested in monitoring the patient for 30 days.
This is what we define as established patients.
RCM 30 DAY COMMITMENT

Reviewing Therapy Algorithms weekly

Document Finding

Contact Patient weekly

Provide encouragement and praise

Address any concerns by educating and communicate findings with the provider

Ensure rapid follow-up and compliance follow has been scheduled.

Contact patient 3 days after rapid follow up.

On Day 30, check usage, educate patient and provide documentation to provider.

Rested Care Management will end contact with patients, IF:

Any negative findings after three weeks

Contact provider to determine if additional 30 days is needed

Consult with provider on an action plan and educate patient accordingly
